Privacy Policy

1. General Provisions

  1. The administrator of the website panmed.com.pl (hereinafter referred to as the “Service”) is PANMED SPÓŁKA Z OGRANICZONĄ ODPOWIEDZIALNOŚCIĄ, KRS 0001186661, NIP 8481892877, REGON 542353546, registered office address: Jana Pawła II 15A, 19-300 Ełk (hereinafter referred to as the “Administrator”).
  2. The Administrator operates a medical facility providing services within the framework of Primary Health Care (POZ) under the National Health Fund (NFZ) system.
  3. Contact with the Personal Data Administrator (ADO) is possible via e-mail at: biuro@panmed.clinic and at the Administrator’s registered office address.
  4. The Administrator attaches particular importance to protecting the privacy of Service users and applies technical and organizational measures to ensure data confidentiality.

2. Personal Data

  1. The panmed.clinic Service currently does not collect or process personal data of Service users. This means that we do not collect data such as first name, last name, e-mail address, or phone number, unless the user voluntarily contacts us using the contact details available in the Service (e.g. e-mail, phone).
  2. In the event of contact via the provided e-mail addresses or phone numbers, personal data (e.g. e-mail address, first and last name) will be processed solely for the purpose of responding to the inquiry and for the time necessary to handle the correspondence.
  3. Note: The processing of patients’ personal data in connection with the provision of healthcare services (registration, appointment, medical documentation) is carried out on the basis of separate legal regulations, including GDPR and laws governing medical activity (e.g. the Act on Patients’ Rights and the Patient Rights Ombudsman), and is regulated by a separate GDPR Information Obligation available at the facility’s registered office.

3. Cookies and Analytical Tools

  1. The Service uses cookies – IT data, in particular text files, stored on Users’ end devices and intended for use of the Service’s websites.
  2. Cookies are used for the following purposes:
    • Adjusting the content of the Service’s websites to the User’s preferences.
    • Conducting anonymous statistical traffic analysis within the Service, which helps understand how Users use the websites and enables improvement of their structure and content.
  3. Use of Google Analytics:
    • The Service uses Google Analytics, an analytical tool provided by Google Ireland Limited (if Google is based in Europe) or Google LLC (if Google is based in the USA) [the appropriate name and address should be provided].
    • Google Analytics uses cookies to analyze how Users use the Service. Information generated by cookies regarding the use of the Service is usually transmitted to and stored on Google servers.

    • Use of the Service involves the use of cookies. In order to ensure legal compliance, the Service should display a transparent notice (so-called cookie banner) requesting consent for the use of cookies, in particular analytical cookies (external ones, such as Google Analytics).
  5. Managing Cookies:
    • The User may limit or disable cookies’ access to their device. If this option is used, use of the Service will remain possible, except for functions that by their nature require cookies.

4. Final Provisions

  1. The Administrator reserves the right to make changes to this Privacy Policy. Changes will each time be published on this subpage of the Service.
